A. INTRODUCTION

SGNHC clinical laboratory is regarded as one of the best laboratories in Nepal. It has adopted latest technologies, state of the art modern instruments and registered professionals.

 

B. STRUCTURE

The Clinical Laboratory is divided into Biochemistry, Hematology, Microbiology and Blood bank sections.

Biochemistry: This unit is equipped with automated analyzers from Dade Behring USA (RXL Max) and Randox UK(RX Daytona). Every day average 200 samples are run, each samples have 6 parameters in average.  Along with the routine test we are doing special tests like Troponin I qunatitative, Lp(a), ADA, HbA1C and Digoxin drug level. Every abnormal report is generally cross checked in parallel. Daily IQC checks are done.

Hematology: Five part laser cell counter along with 3-part coulter counter have made the works of hematology more efficient. We have adopted coagulometers to cope up with increasing number of PT/APTT samples. Daily IQC in coagulometers and at least weekly IQC in hematology analyzer are done.

Microbiology / Serology: Blood c/s and urine c/s are the samples, which are generally received in Microbiology, which is equipped with most of the necessary instrument and reagents. Along with the routine serology we are running ELISA screening for infectious disease, quantification of CRP and Thyroid serology. We are shortly extending the culture ID and AST system into a latest version.

Blood Banking: We produce all major blood components that are required in our centre, which includes Red Cell Concentrate (RCC), Platelet Concentrate (PC) and Fresh Plasma (FFP). We have a fully automated component extractor (TACE-II), Refrigerated centrifuge, and all necessary equipments for components seperation. Our donor station are designed for donors comfort and have a option for a vasovagal attack management.

 

C. SERVICE

We have come over the long waiting for laboratory reports. Dispatch of reports twice daily is the unique feature of SGNHC laboratory. The reports of the entire test are dispatch within the same day. The collection for the investigation is open every day except Saturday from 8:00 AM to 2:00 PM and from 8:00 AM to 12:00 PM even on public holidays. The early collections of the blood samples and holidays have been well appreciated especially by most of the employees in different forms and institutions. The emergency and urgent reports are made available within an hour.

Tests available in SGNHC laboratory

Haematology: Blood Routine (Hb, TC, DLC, ESR, RBC count, Platelets count, PCV, MCV, MCH, MCHC), Peripheral smear, MP, Aldehyde Test, MP serology and Reticulocyte count.

Coagulation: BT, CT, PT, APTT, Fibrinogen and FDP

Biochemistry: Sugar, Urea, Creatinine, Electrolytes (Sodium, Potassium, Calcium and Phosphorus), Uric acid, Bilirubin, Total Protein, Albumin, A/G ratio, Alkaline phosphatase, GPT, GOT, Lipid profile (Total Cholesterol, Triglycerides, HDL, LDL), Cardiac Enzymes ( Troponin I, CPK, CPK (MB), LDH, GOT, ) Amylase, Lp(a), HbA1C, ADA, Digoxin, HsCRP and RA ASO quantitative.

Microbiology: Blood C/S, Urine C/S, Pus C/S, Gram stain, AFB stain

Serology: HIV (ELISA/SPOT), HBsAg (ELISA/SPOT), HCV (ELISA/SPOT), RPR, RA, ASO, CRP

Blood Banking: Blood groping, compatibility testing and blood bag screening

Body Fluids Biochemistry: TC, DC, Na K, LDH, Sp Gravity, Sugar, Protein and ADA

Miscellaneous: Urine R/E M/E, Stool R/E M/E, Occult blood, U. Acetone, Mantoux test

 

D. Quality Management:

Internal quality management: The biochemisty control sera both normal (N) and Abnormal (P) from, Human, Randox, Serodos, and BioRAD are run daily and the results are decisive factor for running the test.

The hematology control sera from Check TM STREACK LABORATORIES, INC, La vista, NEUSA distributed by sysmex cooperatative Kobe Japan are run weekly

The PT/APTT control sera from Dade Behring USA are run daily

 

E. Future Plan:

Automation in the microbiology in detection and isolation. To start Histopathology, Cytopatology, and Bone marrow studies. Seprate emergency,IPD,Lab.
